What the Evidence Shows
Caloric restriction (CR) โ reducing calorie intake by 20-40% while maintaining nutrition โ is the most robust intervention for extending lifespan in laboratory animals across multiple species. Mechanisms include reduced mTOR signaling, enhanced autophagy, reduced oxidative stress, improved insulin sensitivity, and activation of sirtuins. In humans, the CALERIE trial (2-year 25% caloric restriction in healthy non-obese adults) showed improvements in metabolic biomarkers associated with aging: reduced inflammation, improved insulin sensitivity, lower thyroid hormones (associated with longevity in animal models), and reduced oxidative stress. However, we cannot ethically randomize humans to lifelong caloric restriction and wait to see if they live longer. Observational data from long-lived populations (Okinawa) suggests benefit, but is confounded by genetics, diet quality, and lifestyle. The practical challenges are enormous: hunger, muscle loss, reduced bone density, cold sensitivity, reduced libido, and social isolation make sustained 25% restriction very difficult.

Caloric restriction in primates: does it extend lifespan?

In the NIA primate study, rhesus monkeys on 30% caloric restriction did NOT live significantly longer than controls, despite improved metabolic health markers. This contradicted the earlier Wisconsin primate study that showed benefit.
Why this disagrees:
Two long-term primate studies produced conflicting results. The NIA study's control monkeys were leaner and healthier than the Wisconsin controls, suggesting that caloric restriction's apparent longevity benefit may simply reflect avoiding obesity rather than a specific life-extension mechanism.
