What the Evidence Shows
Lion's mane contains compounds called hericenones and erinacines that stimulate nerve growth factor (NGF) synthesis in cell culture and animal studies. NGF is critical for neuron maintenance, survival, and regeneration. In animal models, lion's mane improves spatial memory, reduces amyloid-beta plaque burden, and promotes hippocampal neurogenesis. However, human evidence is extremely limited. The most cited RCT (Mori et al., 2009) found 250mg tablets 3x/day for 16 weeks improved cognitive function scores in elderly Japanese adults with mild cognitive impairment, but effects disappeared 4 weeks after stopping. A few other small trials show modest benefits for mood and subjective cognition. The evidence base is early-stage: most studies are in animals or in vitro, human trials are small (30-80 participants), short-term, and predominantly from Japan. Whether hericenones/erinacines cross the blood-brain barrier in meaningful quantities in humans is still debated.

Reviews of cognitive supplements consistently find that evidence supporting nootropic effects in healthy, cognitively-intact adults is insufficient. Most positive findings come from impaired populations and may not generalize to healthy users.
Why this disagrees:
The existing evidence for lion's mane comes almost exclusively from elderly individuals with existing cognitive decline. There's essentially no strong evidence it enhances cognition in healthy young adults โ the demographic most likely to purchase it. The 'brain-boosting' marketing far exceeds what the science supports for healthy users.
