What the Evidence Shows
The strongest evidence comes from the Finnish KIHD cohort study (2,315 men, 20+ year follow-up), which found a dose-response relationship: those using a sauna 4-7 times per week had 40% lower all-cause mortality compared to once-per-week users. Similar associations exist for sudden cardiac death (63% lower) and cardiovascular disease. The proposed mechanisms include improved vascular function (endothelial dilation), blood pressure reduction, reduced arterial stiffness, enhanced parasympathetic activity, heat shock protein activation, and reduced chronic inflammation. Short-term RCTs confirm acute cardiovascular benefits: blood pressure drops, heart rate increases mimicking moderate exercise, and inflammatory markers decrease. However, no long-term RCT has tested whether prescribing sauna use actually extends life. The observational data may be confounded — frequent sauna users may be healthier overall, more socially connected, and have higher socioeconomic status.
