What the Evidence Shows

Ashwagandha (Withania somnifera) is an adaptogenic herb with withanolide compounds proposed to support testosterone production through cortisol reduction and direct gonadal stimulation. Several RCTs have reported testosterone increases of 10-22% in men taking 300-600mg of root extract (standardized to 5% withanolides) for 8-12 weeks. The most cited mechanism is cortisol-mediated: since cortisol and testosterone share precursor pathways, reducing cortisol may theoretically shunt pregnenolone toward testosterone synthesis. Studies in stressed men and overweight sedentary males show larger effects than those in healthy athletic populations. A 2019 systematic review found consistent but modest testosterone elevations across 5 RCTs, along with improvements in sperm parameters. However, absolute testosterone increases are often within normal variation (50-150 ng/dL), raising questions about functional significance. In young healthy men with normal testosterone, the effect may be negligible. The supplement also appears to improve subjective vitality and exercise performance metrics, which may be independent of testosterone changes. Importantly, the testosterone-boosting effect has not been demonstrated to produce clinically meaningful outcomes like increased muscle mass or improved sexual function beyond what exercise alone provides.
