What the Evidence Shows

Sauna bathing, particularly Finnish-style dry sauna at 80-100°C, produces acute cardiovascular responses similar to moderate exercise: increased heart rate (100-150 bpm), cardiac output, and peripheral vasodilation. The landmark Kuopio Ischemic Heart Disease (KIHD) cohort study followed over 2,300 Finnish men for 20+ years and found that those using saunas 4-7 times per week had a 63% lower risk of sudden cardiac death compared to once-weekly users. Mechanistically, regular heat exposure improves endothelial function, reduces arterial stiffness, lowers blood pressure, and may reduce systemic inflammation. RCTs have demonstrated acute improvements in flow-mediated dilation and blood pressure reduction lasting several hours post-session. Chronic sauna use (3-4 weeks of daily sessions) shows sustained reductions in systolic blood pressure of 5-7 mmHg in hypertensive patients. Heat shock proteins upregulated during sauna exposure may provide cardioprotective effects through improved protein homeostasis and reduced oxidative stress. However, the observational nature of the strongest mortality data means confounding factors (healthy user bias, exercise habits, socioeconomic status) cannot be fully excluded. Interventional trials are shorter and smaller, limiting conclusions about long-term outcomes.
