What the Evidence Shows
Berberine is a plant alkaloid found in goldenseal, barberry, and Oregon grape that activates AMP-activated protein kinase (AMPK), a central metabolic regulator involved in glucose uptake and insulin sensitivity. Dong et al. (2012) published a meta-analysis in Evidence-Based Complementary and Alternative Medicine pooling 14 RCTs showing berberine significantly reduced fasting plasma glucose and was not inferior to oral hypoglycemic drugs when combined with lifestyle interventions. Liang et al. (2019) conducted a comprehensive meta-analysis in Frontiers in Pharmacology confirming berberine's efficacy for reducing both glucose and lipid parameters, with effects on fasting glucose of approximately 0.5-0.7 mmol/L reduction compared to placebo. Zhang et al. (2008) demonstrated in a controlled trial published in Metabolism that berberine (500 mg three times daily) reduced HbA1c from 7.5% to 6.6% over 3 months in type 2 diabetics, with effects comparable to a metformin comparison group.
