What the Evidence Shows

Magnesium L-threonate (MgT) was developed at MIT specifically to cross the blood-brain barrier more effectively than other magnesium forms. Animal studies demonstrated that MgT uniquely elevates brain magnesium levels, enhancing synaptic plasticity, NMDA receptor function, and spatial memory in aging rats. These preclinical results generated significant commercial interest. However, human evidence is extremely limited. Only one peer-reviewed RCT exists: a 2016 study of 44 adults aged 50-70 showing improvements in some cognitive tests (particularly executive function and working memory) after 12 weeks of supplementation. While statistically significant, the small sample size, single-center design, and manufacturer funding limit confidence. Several marketed claims about MgT being superior to other magnesium forms for cognition rely primarily on animal data extrapolation. It remains unclear whether MgT's unique ability to elevate brain magnesium in rodents translates proportionally to humans. Other magnesium forms (glycinate, taurate) may also support cognition through general magnesium repletion in deficient individuals. The gap between animal promise and human proof remains substantial.
