What the Evidence Shows

DHA (docosahexaenoic acid) comprises approximately 40% of polyunsaturated fatty acids in the brain and is critical for membrane fluidity, synaptic function, and neuroinflammation regulation. This biological importance led to widespread supplementation for cognitive enhancement. However, large RCTs and meta-analyses in cognitively healthy adults have been disappointing. The AREDS2 and VITAL cognitive substudies found no benefit of omega-3 supplementation on cognitive decline over 4-5 years in healthy older adults. Younger healthy adults show similarly null results for memory, executive function, and processing speed. The disconnect between DHA's structural importance and supplementation futility likely reflects that healthy adults on typical Western diets maintain adequate brain DHA levels, and additional supplementation cannot enhance already-sufficient neural function. Where benefits appear more consistent is in populations with low baseline omega-3 intake, mild cognitive impairment, or depression comorbidity.
