What the Evidence Shows

Black seed oil, extracted from Nigella sativa seeds, has been used in traditional medicine for centuries. Its primary bioactive compound, thymoquinone (TQ), has demonstrated anti-inflammatory activity through multiple mechanisms: inhibition of NF-kB signaling, suppression of pro-inflammatory cytokines (TNF-alpha, IL-6, IL-1beta), and modulation of COX-2 and lipoxygenase pathways. Clinical evidence includes several RCTs showing reductions in C-reactive protein (CRP), TNF-alpha, and other inflammatory biomarkers in populations with metabolic syndrome, rheumatoid arthritis, and type 2 diabetes. A 2019 meta-analysis of 11 RCTs found significant reductions in CRP (weighted mean difference: -1.52 mg/L) with black seed oil supplementation. However, study quality varies considerably—many trials are small (n<50), conducted in single centers in Middle Eastern countries, and use inconsistent dosing and formulations. The standardization of thymoquinone content across commercial products is poor, making it difficult to compare studies or ensure therapeutic doses.
