Longevity has become Silicon Valley’s favorite obsession. Billionaires are spending millions on blood transfusions, supplement stacks, and hyperbaric chambers. Podcasters debate rapamycin dosing like it’s fantasy football. But beneath the hype, what actually has evidence for extending human lifespan or healthspan?
We’ve ranked the most discussed longevity interventions from strongest to weakest evidence. Note: “evidence strength” here means human evidence — mouse studies alone don’t qualify for top ranking.
Tier 1: Strong Human Evidence
1. Regular Exercise (Especially Cardio)
Evidence strength: Very strong
Nothing comes close to exercise for longevity evidence. Large prospective cohorts consistently show 30-35% reductions in all-cause mortality for active vs. inactive individuals. Even modest activity (150 min/week moderate) captures most of the benefit.
Zone 2 cardio and longevity is particularly well-supported — low-intensity aerobic exercise improves mitochondrial function, cardiovascular efficiency, and metabolic flexibility. The dose-response curve shows benefits up to about 60-75 minutes daily, with diminishing returns beyond that.
2. Not Smoking
Evidence strength: Very strong
Smoking reduces life expectancy by approximately 10 years. Quitting at any age provides benefit, with the greatest gains from earlier cessation. This is the single largest modifiable risk factor for premature death in developed countries.
3. Social Connection
Evidence strength: Strong
Meta-analyses of over 3.4 million participants found that social isolation and loneliness are associated with 26-32% increased mortality risk — comparable to smoking 15 cigarettes per day. The evidence for social connection extending lifespan is robust across cultures and age groups.
This doesn’t get the attention it deserves in longevity circles, perhaps because you can’t bottle it and sell it as a supplement.
4. Adequate Sleep (7-9 Hours)
Evidence strength: Strong
Both short sleep (<6 hours) and long sleep (>9 hours) are associated with significantly increased mortality. The mechanisms are well-characterized: immune dysfunction, cardiovascular strain, impaired glucose metabolism, and accelerated neurodegeneration.
Tier 2: Moderate Human Evidence
5. Diet Quality (Mediterranean-Style)
Evidence strength: Moderate-strong
The Mediterranean diet has the most longevity evidence of any dietary pattern, with the PREDIMED trial showing significant reductions in cardiovascular events. General principles (high fiber, adequate protein, minimal ultra-processed food) are consistently associated with lower mortality.
6. Moderate Alcohol Avoidance
Evidence strength: Moderate-strong
Recent Mendelian randomization studies have dismantled the “J-curve” hypothesis that moderate drinking extends life. The safest level of alcohol for longevity appears to be zero or very low intake. Previous observational associations likely reflected confounders.
7. Sauna Use
Evidence strength: Moderate
Finnish cohort studies show dose-dependent associations between sauna frequency and reduced cardiovascular mortality. Men using sauna 4-7 times per week had 40% lower all-cause mortality than once-weekly users. Plausible mechanisms include improved vascular function, heat shock protein activation, and cardiovascular conditioning.
8. Caloric Restriction
Evidence strength: Moderate (in humans)
The CALERIE trial (25% restriction for 2 years) showed improvements in metabolic markers and biological aging measures. Animal data is extraordinarily consistent across species. However, we lack long-term human lifespan data, and severe restriction carries risks (muscle loss, bone density reduction, immune suppression).

Tier 3: Preliminary or Weak Human Evidence
9. Cold Exposure
Evidence strength: Weak-moderate
Cold exposure activates brown fat, increases norepinephrine, and may improve metabolic health. However, direct longevity evidence in humans is essentially absent. The mechanisms are plausible but the leap from “increases a hormone” to “extends life” requires evidence we don’t yet have.
10. Rapamycin
Evidence strength: Preliminary in humans
Rapamycin extends lifespan in mice more consistently than almost any other intervention. The PEARL trial and other small human studies show immune improvements in older adults. But long-term safety data in healthy humans is lacking, side effects (immunosuppression, metabolic issues) are real, and optimal dosing is unknown.
11. NMN / NR (NAD+ Precursors)
Evidence strength: Weak
Despite enormous consumer enthusiasm, human trials of NMN and NR have shown only modest biomarker changes with unclear clinical significance. The mouse data is interesting but hasn’t translated convincingly to humans yet. Multiple well-designed RCTs have failed to show the dramatic benefits seen in rodent models.
12. Metformin for Non-Diabetics
Evidence strength: Preliminary
The TAME (Targeting Aging with Metformin) trial is underway but hasn’t reported results. Observational data from diabetic populations suggests possible longevity benefits, but confounding is significant. Some evidence suggests metformin may blunt exercise adaptations.
The Uncomfortable Truth
The highest-evidence longevity interventions are boring: exercise, don’t smoke, maintain social connections, sleep well, eat whole foods. These aren’t sexy enough for podcast sponsorships or biohacking conferences.
Most people chasing longevity through exotic interventions would gain more years by consistently implementing the basics. A 50-year-old who starts exercising regularly, improves their diet, and nurtures social relationships is doing more for their lifespan than someone taking rapamycin and NMN while remaining sedentary and isolated.
The Takeaway
Longevity science is legitimately exciting, and interventions like rapamycin may eventually prove transformative. But today, in 2026, the evidence hierarchy is clear: lifestyle factors dominate, and pharmacological interventions remain speculative for healthy humans. Build your foundation before chasing cutting-edge compounds.
