Yoga has been studied as a treatment for chronic low back pain (CLBP) in numerous randomized controlled trials with generally positive but modest results. Wieland et al. (2017) conducted a Cochrane review of 12 RCTs (1,080 participants) comparing yoga to non-exercise controls and found low-to-moderate quality evidence that yoga provides small-to-moderate improvements in back-related function (SMD -0.44) and pain at 3-6 months. However, when compared to other exercise interventions, yoga showed no superiority. Saper et al. (2017) published a well-designed RCT comparing yoga to physical therapy in 320 adults with chronic LBP, finding that yoga was non-inferior to physical therapy for pain and function outcomes at 12 weeks, with both superior to education alone. This suggests yoga's benefits come from the physical activity component rather than specific yogic elements. Cramer et al. (2013) performed a meta-analysis of 10 RCTs finding significant short-term effects on pain (SMD -0.48) and disability (SMD -0.59) compared to usual care. However, Tilbrook et al. (2011) conducted a large pragmatic RCT (n=313) comparing 12 weeks of yoga to usual care for chronic LBP and found that while yoga produced significantly better back function scores at 3 months, the differences were modest and pain outcomes were not significantly different between groups.
