What the Evidence Shows

Passive heat therapy through hot water immersion (hot tubs, hot baths) induces physiological responses similar to moderate exercise: increased heart rate, cardiac output, and peripheral vasodilation. A 2016 Journal of Physiology study demonstrated that 8 weeks of repeated hot water immersion improved endothelial function and reduced arterial stiffness in sedentary young adults—changes comparable to aerobic exercise training. A large Finnish cohort study (2018) found that frequent sauna/bathing was associated with reduced cardiovascular mortality over 20+ years of follow-up. Mechanistically, heat exposure stimulates heat shock proteins, improves nitric oxide bioavailability, and may reduce chronic inflammation. However, a 2016 critique noted that cardiovascular improvements from passive heating are acute and transient, with unclear durability once heat exposure ceases, and that observational associations may be confounded by lifestyle factors.
