What the Evidence Shows
The relationship between sauna bathing and dementia risk gained attention following publication of results from the Kuopio Ischaemic Heart Disease Risk Factor Study (KIHD), a Finnish prospective cohort. Laukkanen et al. (2017) published in Age and Ageing showing that men who used a sauna 4-7 times per week had a 66% lower risk of dementia and 65% lower risk of Alzheimer's disease compared to those using it once weekly, over a 20-year follow-up period. The dose-response relationship was striking and persisted after adjustment for multiple cardiovascular and lifestyle confounders. Laukkanen et al. (2018) expanded on these findings in Mayo Clinic Proceedings, reviewing mechanisms including improved vascular function, reduced inflammation, enhanced heat shock protein expression, increased BDNF, and cardiovascular conditioning that mimics moderate exercise. Kunutsor et al. (2018) conducted a meta-analysis published in BMC Medicine pooling sauna studies with cardiovascular mortality outcomes, finding consistent inverse associations between frequency of sauna use and adverse cardiovascular events. Since cardiovascular health is strongly linked to dementia risk, this provides indirect mechanistic support. However, Zaccardi et al. (2017) examined the acute hemodynamic effects of sauna in the Journal of Human Hypertension and emphasized that while blood pressure decreases acutely during sauna sessions, there is insufficient evidence from interventional studies to confirm long-term health benefits. The observational nature of existing evidence means confounding by overall healthy lifestyle, socioeconomic status, and selection bias (healthier people use saunas more) cannot be excluded. No randomized trials have tested sauna for dementia prevention.
