What the Evidence Shows
The gut-brain axis refers to bidirectional communication between the enteric nervous system and the central nervous system via the vagus nerve, immune signaling, and microbial metabolites (short-chain fatty acids, tryptophan metabolites, and neurotransmitter precursors). Approximately 90% of the body's serotonin is produced in the gut, and microbial composition directly influences its synthesis. Large observational studies consistently identify dysbiotic microbiome profiles in individuals with major depressive disorder (MDD) and generalized anxiety disorder (GAD)—typically showing reduced Lactobacillus, Bifidobacterium, and microbial diversity with increased pro-inflammatory species. Meta-analyses of probiotic supplementation trials (termed psychobiotics) demonstrate small but statistically significant improvements in depression scores (SMD: -0.24 to -0.37) and anxiety symptoms. The most studied strains include Lactobacillus rhamnosus, Bifidobacterium longum, and multi-strain formulations.
