A 2019 Canadian birth cohort study (MIREC) reported that maternal fluoride exposure during pregnancy was associated with lower IQ in boys but not girls, sparking significant debate. The NTP (National Toxicology Program) conducted a systematic review concluding that fluoride is 'presumed' to be a cognitive neurodevelopmental hazard at exposures above 1.5 mg/L, but found inadequate evidence for effects at 0.7 mg/L. Critics note that many supporting studies come from regions with uncontrolled confounders (arsenic co-exposure, poverty, malnutrition) and use ecological rather than individual-level exposure assessment. A 2024 federal court ruling ordered the EPA to further evaluate risks, but did not establish that fluoride at recommended levels causes harm.
