What the Evidence Shows
The relationship between fish consumption and stroke risk has been evaluated in several large meta-analyses of prospective cohort studies. Xun et al. (2012) analyzed 16 prospective studies and found that each additional serving of fish per week was associated with a 2% lower risk of stroke, with a plateau at approximately 3-5 servings per week. He et al. (2004) similarly found a dose-response relationship, with those consuming fish 5 or more times per week having 31% lower stroke risk compared to those consuming fish less than once per month. Larsson and Orsini (2011) confirmed a non-linear dose-response, with maximum benefit at 2-4 servings per week. The proposed mechanism involves omega-3 fatty acids (EPA and DHA) reducing blood pressure, triglycerides, platelet aggregation, and inflammation. However, important limitations exist. All major evidence comes from observational studies where fish consumption is a marker of overall healthier dietary patterns and socioeconomic status. The DART-2 trial (Burr et al. 2003) raised concerns by paradoxically finding increased mortality in men advised to eat oily fish or take fish oil capsules after myocardial infarction, though the reasons remain debated. Confounding by overall diet quality, physical activity, and socioeconomic factors cannot be fully excluded in observational studies.
