The popular belief that alcohol 'kills brain cells' is an oversimplification. Landmark neuropathological studies by Harper and Kril comparing alcoholic and control brains found that neuron counts in the cerebral cortex were not significantly reduced in uncomplicated alcoholics—the apparent brain shrinkage was attributable to white matter loss and dendritic retraction rather than neuronal death. However, this does not mean moderate alcohol is benign for the brain. Large neuroimaging studies including UK Biobank (n=25,378) demonstrate a clear dose-response relationship between alcohol consumption and reduced total brain volume, with even moderate drinking (7-14 units/week) associated with measurably smaller brain volumes equivalent to 1-2 years of aging.
