What the Evidence Shows

Cinnamon contains cinnamaldehyde and type-A proanthocyanidins that have shown insulin-sensitizing properties in vitro by enhancing insulin receptor phosphorylation and increasing glucose transporter (GLUT4) translocation. Some meta-analyses report fasting glucose reductions of 3-24 mg/dL in type 2 diabetics, but heterogeneity is extreme (I-squared often exceeding 80%). The type of cinnamon matters: Ceylon cinnamon (Cinnamomum verum) has different bioactive profiles than Cassia cinnamon (C. cassia), yet many studies fail to specify the species used. Additionally, Cassia cinnamon contains coumarin, which poses hepatotoxicity risk at high doses. Studies showing positive effects tend to be conducted in poorly controlled diabetic populations where any intervention might show marginal benefit. In well-controlled diabetics and pre-diabetics, effects are typically non-significant. The clinical relevance of a 3-5 mg/dL fasting glucose reduction is questionable given the natural daily variation in blood glucose levels. HbA1c reductions are minimal (0.06-0.16%) and rarely reach clinical significance.
