Observational studies consistently show an association between low vitamin D levels and depression, but this may reflect reverse causation (depressed people go outside less). RCTs of vitamin D supplementation show mixed results: some meta-analyses find small benefits for depressive symptoms, particularly in those with baseline deficiency, while others find no significant effect. The largest and most rigorous trials (including VITAL-DEP) found no benefit for preventing depression in vitamin D-sufficient adults. Vitamin D may play a supporting role in mood regulation through serotonin synthesis and neuroinflammation pathways, but current evidence does not support it as a primary treatment.
