What the Evidence Shows

The relationship between vitamin D and muscle function has been investigated extensively, with earlier meta-analyses showing positive effects that have not been consistently replicated in more rigorous recent trials. Beaudart et al. (2014) published a meta-analysis in the Journal of Clinical Endocrinology and Metabolism analyzing 30 RCTs and found a small but statistically significant positive effect of vitamin D supplementation on muscle strength, particularly in the lower extremities, with greater effects in individuals with baseline 25(OH)D levels below 30 nmol/L. Tomlinson et al. (2015) conducted a meta-analysis in the Journal of Strength and Conditioning Research confirming that vitamin D supplementation improved upper and lower body strength compared to placebo, though effect sizes were modest. Stockton et al. (2011) in Osteoporosis International also reported that supplementation improved hip flexor and knee extensor strength in deficient populations. The vitamin D receptor is expressed in skeletal muscle tissue, and deficiency causes type II muscle fiber atrophy, impaired calcium handling, and proximal myopathy—providing biological plausibility. However, Grübler et al. (2019) published results from the Styrian Vitamin D Hypertension Trial in the American Journal of Clinical Nutrition, a well-powered RCT giving 2800 IU/day vitamin D3 to vitamin D-insufficient adults. Despite achieving sufficient vitamin D levels, supplementation did not improve handgrip strength, chair-rise test performance, or other muscle function outcomes compared to placebo over 8 weeks. This and other recent trials suggest that earlier positive meta-analyses were influenced by studies in severely deficient populations and that supplementation provides no muscular benefit in mildly deficient or sufficient individuals—the majority of supplementation candidates.

Why this disagrees:

When vitamin D levels are raised from insufficient to sufficient (rather than from severely deficient), there is no measurable improvement in muscle function. The earlier positive meta-analyses may have been driven by correction of severe deficiency, which is less relevant to the majority of supplementation candidates.
