Does a vegan diet reduce heart disease risk?

What the Evidence Shows
Vegan diets eliminate the primary dietary sources of saturated fat and cholesterol (meat, dairy, eggs), which mechanistically should reduce cardiovascular risk. Meta-analyses of observational studies show vegans have 15-25% lower cardiovascular mortality than omnivores, and intervention trials demonstrate meaningful reductions in LDL cholesterol (-15-25%), total cholesterol, blood pressure, BMI, and inflammatory markers like CRP. The Adventist Health Study-2 (96,000 participants followed for 5+ years) found vegans had 42% lower risk of ischemic heart disease compared to non-vegetarians. However, significant caveats exist. Vegans in observational studies tend to be health-conscious overall (more exercise, less smoking, higher education), creating substantial healthy-user bias. No randomized controlled trial has followed vegans long enough to measure actual cardiovascular events (heart attacks, strokes)—only risk factor changes. The quality of a vegan diet matters enormously: a whole-food plant-based diet rich in vegetables, legumes, nuts, and whole grains differs vastly from a processed vegan diet high in refined carbohydrates and industrial seed oils. Additionally, potential nutrient deficiencies in poorly planned vegan diets (B12, omega-3 DHA, iron) could theoretically increase homocysteine and impair vascular function. The evidence supports that a well-planned vegan diet reduces cardiovascular risk factors, but the magnitude of actual event reduction remains unquantified by rigorous trials.
