What the Evidence Shows
The hypothesis that spicy food consumption may extend lifespan has gained attention from several large prospective cohort studies showing consistent inverse associations with mortality. The 2015 BMJ study by Lv and colleagues, following 487,375 Chinese adults for a median of 7.2 years, found that those consuming spicy food almost daily had a 14% lower relative risk of death compared to those eating spicy food less than once weekly. This association was consistent across causes of death (cancer, heart disease, respiratory disease) and appeared independent of other dietary and lifestyle factors. Chopan and Littenberg found similar results in a US NHANES cohort, with chili pepper consumers having 13% lower all-cause mortality. Bonaccio and colleagues in the Moll-i-SANI Italian cohort confirmed the association, finding regular chili pepper use associated with 23% lower all-cause mortality over 8 years. The proposed biological mechanisms center on capsaicin, the active compound in chili peppers: capsaicin activates TRPV1 receptors involved in thermogenesis and energy homeostasis, has demonstrated anti-inflammatory and antioxidant properties, may improve lipid profiles and gut microbiome composition, and has shown anti-obesity effects in animal models. However, all human evidence is observational, and confounding by overall dietary patterns (spicy food consumers may eat more diverse, plant-rich diets), cultural factors, and healthy-user bias cannot be excluded.
