What the Evidence Shows
Plant-based diets have been consistently associated with lower blood pressure in both observational and interventional research. Yokoyama et al. (2014) published a landmark meta-analysis in JAMA Internal Medicine analyzing 7 controlled trials and 32 observational studies, finding that vegetarian diets were associated with significantly lower systolic blood pressure (mean reduction of 4.8 mmHg) and diastolic blood pressure (2.2 mmHg) compared to omnivorous diets. These effect sizes are clinically meaningful—comparable to single antihypertensive medications. Lee et al. (2020) conducted an updated meta-analysis in the Journal of Hypertension confirming these findings across newer trials, demonstrating that various plant-based dietary patterns (vegan, lacto-ovo vegetarian, and plant-predominant) all reduced blood pressure relative to conventional diets. Joshi et al. (2020) published a comprehensive review in the American Journal of Medicine outlining the mechanisms: plant-based diets are typically higher in potassium, magnesium, fiber, and polyphenols; lower in sodium and saturated fat; promote weight loss; improve endothelial function through increased nitric oxide availability; and reduce visceral adiposity and insulin resistance. However, Schwingshackl et al. (2017) published a network meta-analysis in the American Journal of Clinical Nutrition comparing multiple dietary patterns head-to-head and found that the DASH diet (Dietary Approaches to Stop Hypertension)—which includes lean meats, poultry, and dairy—was among the most effective for blood pressure reduction, performing comparably to strict vegetarian approaches. This suggests that the blood pressure benefits may derive from specific dietary components (high potassium, low sodium, high fiber, abundant fruits and vegetables) rather than the exclusion of animal products per se. A well-designed omnivorous diet can match the blood pressure benefits of a plant-based approach.
