What the Evidence Shows

Oregano essential oil (Origanum vulgare) contains carvacrol (60-80%) and thymol (5-20%) as primary bioactive compounds, both of which exhibit well-characterized antibacterial mechanisms in laboratory settings. These phenolic compounds disrupt bacterial cell membrane integrity, increase permeability, and cause leakage of cellular contents. In vitro studies consistently demonstrate minimum inhibitory concentrations (MICs) effective against Staphylococcus aureus, Escherichia coli, Pseudomonas aeruginosa, Klebsiella pneumoniae, and Salmonella species, often comparable to conventional antibiotics at equivalent concentrations. Some studies show activity against antibiotic-resistant strains including MRSA. The antibacterial mechanism is non-specific (membrane disruption), which theoretically reduces resistance development. However, critical gaps exist between laboratory and clinical evidence. Very few human clinical trials have tested oregano oil for bacterial infections, and those that exist are small, poorly controlled, and typically study gut pathogens or upper respiratory infections. Bioavailability, tissue distribution, and achievable concentrations in human tissues may be far below effective in vitro MICs. Additionally, the dose required for systemic antibacterial effects may cause gastrointestinal irritation or hepatotoxicity. Oregano oil clearly has antibacterial properties as a chemical compound, but its clinical utility as an antimicrobial treatment in humans remains unproven.
