What the Evidence Shows

Expressive writing, a paradigm developed by James Pennebaker in the 1980s, involves writing about one's deepest thoughts and feelings regarding stressful or traumatic experiences for 15-20 minutes over 3-5 consecutive days. Smyth (1998) published a meta-analysis in the Journal of Consulting and Clinical Psychology examining 13 studies and found a significant overall effect (d=0.47) of expressive writing on health outcomes, with effects particularly strong for physiological markers (immune function, physician visits) and less consistent for psychological distress measures. Frattaroli (2006) conducted a larger meta-analysis of 146 studies in Psychological Bulletin, confirming a significant but smaller overall effect (r=0.075), noting that effects were moderated by study design, population, and outcome type. Physical health, psychological well-being, physiological functioning, and general functioning all showed significant improvements. Baikie and Wilhelm (2005) published a comprehensive review in Advances in Psychiatric Treatment summarizing the evidence: documented benefits include fewer physician visits, improved immune function (T-helper cell counts, antibody response to vaccines), reduced blood pressure, improved liver function, fewer days in hospital, improved mood, and reduced depressive symptoms.
