What the Evidence Shows
Iodine is essential for thyroid hormone synthesis, which plays a critical role in fetal brain development, myelination, neuronal migration, and synaptogenesis. Severe iodine deficiency during pregnancy causes cretinism, characterized by profound intellectual disability, deaf-mutism, and motor spasticity. Even mild-to-moderate maternal iodine deficiency impairs offspring cognitive development, as demonstrated in multiple prospective cohort studies and natural experiments following iodization programs. A definitive meta-analysis by Qian et al. (2005) analyzing 37 studies found that iodine-deficient populations had IQ scores averaging 12.45 points lower than iodine-sufficient populations. The Avon Longitudinal Study of Parents and Children (ALSPAC) demonstrated that even mild iodine deficiency in UK pregnant women (urinary iodine <150 μg/L) was associated with lower verbal IQ and reading scores in their children at ages 8-9. Intervention studies provide causal evidence: iodine supplementation before or during early pregnancy in deficient populations improves offspring cognitive scores, while supplementation after the first trimester shows diminished benefit, highlighting a critical developmental window. Why this disagrees:
In populations with only mild deficiency and otherwise adequate nutrition, compensatory thyroid mechanisms may maintain sufficient T4 for fetal brain development. The relationship between iodine deficiency and IQ may be non-linear, with cognitive effects emerging only below a severity threshold not reached in this relatively well-nourished population.
