What the Evidence Shows
The relationship between glycemic load and acne has been investigated through several randomized controlled trials and systematic reviews. A landmark 2007 RCT published in the American Journal of Clinical Nutrition assigned 43 male acne patients to either a low glycemic load diet or a conventional high glycemic diet for 12 weeks. The low glycemic group showed significant improvement in acne lesion counts, reduced weight, improved insulin sensitivity, and decreased free androgen levels compared to controls. A 2012 Korean RCT replicated these findings, showing low glycemic diet significantly reduced acne lesion counts and decreased sebaceous gland size on histology. The proposed mechanism is compelling: high glycemic foods cause rapid insulin and insulin-like growth factor 1 (IGF-1) elevation, which increases androgen bioavailability by reducing sex hormone-binding globulin, stimulates sebocyte proliferation and sebum production, and promotes follicular hyperkeratinization. This pathway links the Western high-glycemic diet to the hormonal cascade driving acne pathogenesis. Epidemiologically, populations consuming traditional low-glycemic diets (Kitavan Islanders, Ache hunter-gatherers) have virtually no acne. However, critical limitations persist. The number of RCTs is small, blinding is impossible in dietary interventions, and compliance verification is challenging. A 2016 narrative review concluded that while the theoretical framework is plausible and preliminary evidence supportive, the evidence base remains insufficient to make strong clinical recommendations, and confounding by dairy, overall caloric intake, and other dietary factors cannot be excluded.
