Attention-deficit/hyperactivity disorder (ADHD) is characterized by difficulties with attention, hyperactivity, and impulsivity. Exercise has been proposed as a complementary intervention due to its effects on dopaminergic and noradrenergic systems, which overlap with ADHD medication targets. Vysniauske et al.'s 2020 meta-analysis found that both acute and chronic exercise interventions improved attention, executive function, and hyperactivity/impulsivity in individuals with ADHD, with moderate effect sizes. Hoza et al. (2015) demonstrated in a school-based RCT that 12 weeks of physical activity before school reduced ADHD symptoms and improved peer behavior compared to sedentary controls. Pontifex et al. (2013) found that a single bout of moderate-intensity exercise improved cognitive performance and inhibitory control in children with ADHD. The proposed mechanisms include increased catecholamine release (dopamine and norepinephrine), enhanced cerebral blood flow, upregulation of brain-derived neurotrophic factor (BDNF), and structural changes in prefrontal regions. Most studies are acute (single-session) rather than chronic, sample sizes tend to be small, and there is heterogeneity in exercise types and intensities used. Additionally, exercise effects are generally smaller than medication effects and likely serve as a complement rather than replacement for standard ADHD treatment.
