What the Evidence Shows

Dark chocolate rich in cocoa flavanols (particularly epicatechin) has demonstrated acute and chronic cardiovascular benefits in controlled trials. The primary mechanism involves flavanol-mediated increases in nitric oxide bioavailability, leading to vasodilation and improved endothelial function. Meta-analyses of RCTs consistently show reductions of 2-4 mmHg in systolic blood pressure and 1-2 mmHg in diastolic pressure, improvements in flow-mediated dilation (a marker of endothelial function), and modest reductions in LDL cholesterol. Large prospective cohort studies have reported 10-20% lower cardiovascular event risk in higher chocolate consumers. However, several limitations temper enthusiasm. Commercial dark chocolate varies enormously in flavanol content depending on processing. Most positive RCTs used specially prepared high-flavanol products not representative of retail chocolate. The caloric density (150-170 kcal per 30g) means habitual consumption contributes to energy surplus. The COSMOS trial (21,442 participants) tested cocoa flavanol supplements rather than chocolate, finding a modest reduction in cardiovascular events over 3.6 years. This separates the flavanol effect from the chocolate matrix, suggesting supplements may be more practical than dietary chocolate for cardiovascular benefit.
