What the Evidence Shows
Curcumin, the active polyphenol in turmeric, has been investigated for antidepressant effects based on its anti-inflammatory and neuroprotective properties. Ng et al. (2017) conducted a meta-analysis in the Journal of Affective Disorders examining 6 RCTs and found curcumin significantly reduced depressive symptoms compared to placebo, with effects most pronounced when used as adjunctive therapy alongside conventional antidepressants. Lopresti et al. (2014) published a well-designed RCT in the Journal of Affective Disorders showing 1000 mg/day of a bioavailability-enhanced curcumin (BCM-95) significantly improved depression scores compared to placebo over 8 weeks in major depressive disorder. Al-Karawi et al. (2016) corroborated these findings in their meta-analysis published in the Journal of the Neurological Sciences, reporting significant pooled effects favoring curcumin over placebo. The proposed mechanisms include reduction of neuroinflammatory cytokines (IL-6, TNF-alpha), modulation of the HPA axis, increased BDNF levels, and monoamine neurotransmitter regulation. However, Andrade (2014) raised significant concerns in an Indian Journal of Psychiatry editorial, noting that positive trials had small sample sizes (n=20-56), short durations (4-8 weeks), high heterogeneity in curcumin formulations and dosages, and that the clinical significance of statistical improvements remained unclear. Curcumin's notoriously poor bioavailability means that most ingested curcumin never reaches systemic circulation, and different formulations (piperine-enhanced, liposomal, nanoparticle) produce vastly different plasma levels, making cross-study comparisons difficult. The evidence is promising but preliminary.
