The theoretical basis for immune enhancement is straightforward: these bioactive components could survive digestion and interact with gut-associated lymphoid tissue (GALT) or provide passive immunity in the gastrointestinal tract. However, the translation from compositional analysis to clinical immune benefit in humans has been disappointing. While some studies in athletes report reduced incidence and duration of upper respiratory tract infections (URTI), these findings are not consistent across trials and effect sizes are modest. A key limitation is that most bovine IgG is specific to bovine pathogens, not human pathogens, limiting direct antimicrobial activity. Lactoferrin may provide non-specific immune modulation, but circulating levels do not significantly increase with oral colostrum supplementation, suggesting limited systemic absorption. The gut-level effects—maintaining mucosal integrity during exercise stress and supporting secretory IgA production—represent the most plausible mechanism but lack consistent clinical validation. Most positive trials are small, industry-funded, and use self-reported illness outcomes rather than laboratory-confirmed infections.
