What the Evidence Shows
Cold water immersion and swimming have gained popularity as potential mood-enhancing interventions, with practitioners reporting euphoria, improved mental clarity, and reduced depressive symptoms. Massey et al. (2020) published a notable case report in BMJ Case Reports describing a patient who achieved remission from treatment-resistant major depression following a program of weekly cold water swimming, allowing discontinuation of medication. Shevchuk (2008) proposed a mechanistic hypothesis that cold exposure activates the sympathetic nervous system, increases beta-endorphin and noradrenaline levels, and stimulates peripheral cold receptors that send electrical impulses to the brain, potentially producing antidepressant effects. Van Tulleken et al. (2018) documented similar clinical improvements through cold water swimming as an adjunct intervention for depression. The physiological stress response to cold water (cold shock response) triggers massive sympathetic activation, endorphin release, and subsequent parasympathetic rebound, which may explain the mood elevation reported after immersion. Regular exposure may also build stress resilience through repeated hormetic stress adaptation. However, the evidence base is extremely limited—consisting primarily of case reports, cross-sectional surveys of swimmers, and small uncontrolled studies. No adequately powered RCTs have been conducted. Confounding is substantial: cold water swimmers also exercise, spend time outdoors, socialize, and self-select for adventurousness. The placebo/expectancy effect of an extreme novel experience is impossible to control for. Additionally, cold water immersion carries real risks including cold shock drowning, cardiac arrhythmias, and hypothermia.
