What the Evidence Shows
Several RCTs have demonstrated that cocoa flavanol supplementation (500-900 mg/day) can improve cognitive performance, particularly executive function and processing speed, in older adults. A landmark 2014 Nature Neuroscience study showed high-flavanol cocoa enhanced dentate gyrus function and improved pattern separation memory in healthy older adults via fMRI. Mechanistically, flavanols improve cerebrovascular function by increasing cerebral blood flow and promoting endothelial nitric oxide synthesis. A 2012 Italian RCT found dose-dependent improvements in cognitive function alongside blood pressure reduction in elderly subjects with mild cognitive impairment. However, a 2013 systematic review concluded that while acute improvements in some cognitive tasks are demonstrable, the evidence for sustained cognitive benefit is inconsistent, with publication bias and heterogeneous outcome measures limiting conclusions.
